Job Title: Long Term Care Nurse Specialist

As a dedicated and compassionate Long Term Care Nurse Specialist, you will be responsible for providing high-quality patient care to residents in long term care settings. Your primary goal will be to ensure the health, comfort, and safety of residents while maintaining a positive and respectful environment.

Key Responsibilities:

  • Provide professional nursing care to residents under the supervision of a registered nurse, advanced practice provider, or physician.
  • Conduct thorough assessments of residents' physical and psychosocial needs to develop individualized care plans.
  • Administer medications, treatments, and perform testing as ordered by a licensed practitioner.
  • Counsel residents, family members, and/or resident advocates on prevention and treatment plans.
  • Collaborate with healthcare professionals to provide comprehensive care and support services.


Requirements:
  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
