In the MTM Pharmacist Independent Contractor role, pharmacists provide patient outreach and medication therapy management services to patients around the country. Join us and enjoy a 100% remote, flexible work opportunity that fits into your life as you see fit.In this role, the MTM Pharmacist Independent Contractor will:Outreach to patients - Make and receive phone calls with patients as an extension of a pharmacy, health plan, pharmacy benefits manager, or health system, many of whom may not have regular interaction with a pharmacist as part of their care team.Offer expert support and guidance - Engage with patients to provide defined clinical services such as comprehensive medication reviews (CMRs), targeted medication reviews (TMRs), medication reconciliation, prior authorization review, and other clinical services. Answer all questions around specific issues around common medical needs (i.e. diabetes, asthma, smoking cessation, high blood pressure). Counsel and train patients on proper use of medical devices and supplies. Partner with healthcare professionals to review the quality and effectiveness of drugs, providing advice on usage and observed side effects.Assess medication therapy - Provide medication therapy management (MTM) services on behalf of contracted clients to assess the appropriateness of medication therapy, improve clinical outcomes, manage medication costs, and identify unnecessary medications or additional medication needs. Identify adherence concerns and provide solutions for patients to get the most from their medications.Document your efforts - Produce documentation of your clinical recommendations, responses to therapy, and patient care notes in a clear, concise, timely, professional manner. RequirementsCandidates for the MTM Pharmacist Independent Contractor role must have:The ability to speak with medical fluency in English.A Doctor of Pharmacy (Pharm.D.) or Bachelor of Science in Pharmacy (BS Pharm) degree.An active pharmacist license to practice in Massachusetts.The ability to obtain licensure in additional states as needed.Strong telephonic skills to service an elderly population in a clear and fluid manner.Excellent communication, listening, and interpersonal skills with the ability to develop patient rapport quickly and effectively.A professional attitude and behavior and a strong work ethic.The ability to thrive working independently as well as in collaboration with a virtual/decentralized team.The ability to self-motivate and multitask.Strong attention to detail and organizational skills.Proficiency using Microsoft productivity applications such as Teams, Outlook, Word, Excel, etc.Preferred QualificationsIdeal candidates for the MTM Pharmacist Independent Contractor role will have: experience conducting Medication Therapy Management services, including knowledge of MTM services such as comprehensive and targeted medication therapy reviews, personal medication lists, and medication action plans.Certification by the Board of Pharmaceutical Specialties (BCPS, BCGP, etc.), the National Board of Medication Therapy Management (BCMTMS), or other applicable certifications in areas such as diabetes, geriatrics, opioid management, or medication therapy management.Sales or call center experienceMinimum AvailabilityCustom Health is looking for contractors who can commit to performing services an average of at least 20 hours per week. Our flexible work environment allows you to choose when and how long you work at any time during our hours of operation, which are Mon-Sat 9am - 7pm and Sun 12pm - 7pm in our patients' local time zones nationwide.Equipment NeededA Windows or Apple Mac laptop or desktop computerReliable high-speed internet serviceA headset with microphone, preferably connected to your computer by wireLocationThis is a fully remote, work-from-home position.
