Responsibilities

  • Monitor vital signs remotely and assess the need for intervention by healthcare providers.
  • Build relationships with patients for engagement, support, and communication.
  • Remotely monitor enrolled patients through daily review of electronic patient data.
  • Provide education and tools to support improved patient outcomes.

  • Obtain consent from patients opting into the programs.
  • Troubleshoot devices and report malfunctions as necessary.
  • Answer routine inquiries and relay information to and from appropriate health professionals.

Requirements

  • Bachelor's degree (preferred)
  • 1 year of experience as a Certified Nursing Assistant (CNA)
  • 2-5 years of related professional or healthcare medical experience
  • Experience in RPM/CCM care management (3 years required)
  • Proficient in Microsoft Excel and Google Sheets
  • Excellent oral and written communication skills
  • Ability to manage multiple tasks simultaneously
  • Self-motivated and able to work independently and as part of a team
  • Experience using EHR systems

Nice-to-haves

  • LPN/CNA/Medical Assistant certification
