Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including long-term care, assisted living, and acute care. As an LPN, you will play a vital role in supporting patients’ health and well-being while collaborating with a multidisciplinary team to deliver exceptional medical services.

Responsibilities

  • Provide direct patient care, including administering medications and treatments as prescribed by physicians.
  • Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
  • Assist with daily living activities and support patients with mobility challenges.
  • Maintain accurate medical documentation and records in compliance with HIPAA regulations.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Educate patients and their families on health management, medication administration, and preventive care.
  • Participate in triage and emergency response as needed.
  • Ensure a safe and clean environment for patients through adherence to infection control protocols.

Qualifications

  • Current LPN license in the state of practice.
  • Experience in critical care, long-term care, or assisted living preferred.
  • Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth is a plus.
  • Knowledge of ICD-9/ICD-10 coding and medical terminology is beneficial.
  • Strong skills in patient assessment, medication administration, and basic life support.
  • Experience working with diverse populations, including individuals with developmental disabilities or Alzheimer’s disease.
  • Ability to perform procedures such as IV insertion, catheterization, phlebotomy, and specimen collection/processing.
  • Excellent communication skills and the ability to work effectively within a team-oriented environment.
